Connie Moreno-Peraza Named New Director of Santa Cruz County Health Services Agency
The County of Santa Cruz has appointed Connie Moreno-Peraza, MSW, LCSW, as the new Director of the Health Services Agency (HSA), effective September 1, 2025. A seasoned health care leader with a career spanning multiple California counties, Moreno-Peraza brings expertise in behavioral health, equity-focused care, and public sector innovation.
Most recently, Moreno-Peraza served as the Assistant Director for Behavioral Health at the Ventura County Health Care Agency. Her prior leadership roles include Director of Behavioral Health Services in Madera County, where she oversaw a wide-ranging system transformation effort. Her experience also includes senior executive positions in Marin, Napa, San Diego, and Stanislaus counties, managing complex health care systems and multimillion-dollar budgets.
“Connie brings exceptional experience leading health care systems throughout California and a strong commitment to equity and community-centered care,” said County Executive Officer Carlos Palacios. “Her leadership will be vital as we continue to strengthen health services for all Santa Cruz County residents.”
A licensed clinical social worker and bilingual Spanish speaker, Moreno-Peraza has been recognized for her dedication to culturally competent, accessible services—particularly in underserved communities. Her work includes advancing mental health and substance use treatment, integrating public health and behavioral health services, and championing equity initiatives across diverse populations.
“It’s an honor to serve the people of Santa Cruz County,” Moreno-Peraza said. “I look forward to building on the great work already being done and partnering with staff and stakeholders to strengthen our systems of care and create an integrated Health Services Agency.”
Moreno-Peraza holds a Master of Social Work and a Bachelor of Arts in Psychology from California State University, Fresno. She is credentialed as a California County Senior Executive by the CSAC Institute for Excellence in County Government and has been widely recognized for her work in cultural competence and community engagement.
Her appointment signals a continued commitment by the County to deliver equitable, effective, and community-focused health care services across Santa Cruz.
